Send questionnaires to your athletes
Ask for an athlete's health, history and goals, or a weekly check-in. They answer in TrainerPlan and the answers stay in their profile.
Questionnaires let you ask your athletes the same questions every time: an anamnesis when they join you, a health screen before you start, or a check-in every week. They answer in TrainerPlan, the answers stay in the athlete's profile, and TrainerPlan's AI takes them into account when it plans for that athlete or answers questions about them.
Your questionnaires
Go to Library and open the Questionnaires tab. Three templates are ready to send:
- Anamnesis: goals, target event, training history, injuries, medical conditions, medication, sleep and stress (15 questions).
- Health screen: yes/no health questions before you start training together (8 questions). Its intro asks the athlete to talk to their doctor before training more if they answer "Yes" to any of them.
- Weekly check-in: how the week felt, fatigue, sleep, pain and next week's availability (6 questions).
The templates are in your language. Use the pencil to edit a questionnaire, the copy icon to duplicate it and the bin to delete it. Questionnaires you have already sent keep their questions and answers when you edit or delete the form. Click New form to start a blank one.

Create or edit a questionnaire
The editor opens with the form's name and the intro your athlete reads first. Below, each question has its text, its answer type and a Required box. The answer types are:
- Short answer and Paragraph, for text
- Single choice, Multiple choice and Dropdown, with your own options
- Number, Scale 1–10, Yes / No and Date
- File upload, for a medical report or a blood test (PDF or image, up to 20 MB)
Drag a question by its handle to move it, or use the icons on its right to duplicate or remove it. + Add question adds one at the end. On wide screens, the Summary on the left counts the questions, the required ones and the conditional ones: click a question there to jump to it, or drag it to change the order.

Follow-up questions
A question that comes right after a Yes / No question can be asked only when the answer is "Yes": tick Only ask when the previous question is "Yes" under it. In the Anamnesis, for example, "Which injury, and when? How is it now?" only appears to athletes who had an injury.

Sending options
At the end of the editor, under Sending:
- Send automatically when an athlete joins you: every new athlete gets it as soon as they join you, as a notification and by email. Useful for the Anamnesis.
- Assistant coaches can read the answers: the assistant coaches in your team see the answers of the athletes they manage.
- Ask for consent before the athlete answers: the athlete agrees that you, your assistant coaches and TrainerPlan's AI can use the answers. Keep it on for health questions.
Click Save when you are done.

Send a questionnaire to an athlete
- Open the athlete, go to the Profile tab and click Questionnaires in the menu on the left.
- Click Send questionnaire and choose the questionnaire. The eye button next to it opens a preview of what your athlete sees; nothing you fill in there is saved.
- If you want, set a Due date and write a Message. The message shows above the questions.
- Leave Also send it by email ticked to email it too, and click Send.
Your athlete gets a notification straight away and, if you set a due date, a reminder the day before.

Follow the answers
The athlete's Questionnaires page lists everything you sent them, with the date it was sent, the due date and the status: Pending, Answered, or Overdue once the due date has passed. The ones sent automatically say "when they joined".

Click a questionnaire, or its eye icon, to read the answers. You also see whether and when the athlete gave consent, and you can open the files they uploaded.
- Download PDF saves the answers as a PDF.
- Send again opens the send panel with the same questionnaire, for example for the next weekly check-in.
- The bin in the list removes the questionnaire from the athlete, together with their answers.
You only see the answers once the athlete sends them. Each time, you get a notification and an email. To stop the email, go to My Profile › Notifications and untick the email for When an athlete answers a questionnaire.

The AI uses the answers
When you create a plan with the AI or ask the AI Assistant about an athlete, it takes into account their answers to the template questions: goal and target event, the days and hours they can train, injuries, medical conditions, medication, sleep, stress and the weekly check-in. This also works in the forms you made from a template, even if you change the wording. The questions you add yourself are just for you.
